Registration Forms and Waiver

• To download child registration and waiver form, see below.  Both forms must be completed and sent back with payment before the registration will be processed.
• To download adult registration and under 18 waiver form, see below.  Both forms must be completed and sent back with payment before the registration will be processed.
• To download coaching application, see below.  

Coaches must also complete a police check form.  You must go to your local police department to pick up the form.  Residents of the Town of Perth must go to the Town Police Department.  Residents outside of the Town of Perth must go to their local Ontario Provincial Police Detachment.  Police checks are required for all individuals over the age of 18 and are provided at no cost.

Completed forms may also be dropped off at the Canadian Tire Gas Bar.   Please do not leave cash at the Gas Bar, cheques or money orders only.
